ORDERING YOUR REPEAT PRESCRIPTIONS

You can order your repeat prescription using any of the following options:-

Order online. It’s easy to register - registration form can be found under prescriptions tab.

Phone the surgery at any time (24 hour phone line) and select option 1 to leave your repeat prescription request.

In person or by post. There is a mail box at front of surgery which can be accessed at any time.

 

To Register with the Practice

PLEASE ATTEND THE SURGERY TO COLLECT ALL THE PAPERWORK WE REQUIRE FOR REGISTRATION.

Patients are eligible to apply to be registered with the Practice if over 16, and living in the practice area.  You will be asked to complete a registration form and a new patient questionnaire.  You will be asked to produce two forms of identification.

Applications may be made on behalf of any child or any adult who is incapable of making such an application by, in the case of a child, either a parent, or in the absence of both parents, the guardian or other adult person who has care of the child, and in the case of an adult, by the primary carer of that person or a person authorised to act on the patients behalf.
